2. Wait for Approval. If request is approved continue to step 3. Approval will be sent via the email address that was provided.

3. Download a copy of the Region 5 Equipment Use Form PDF from the link below.

4. Print and sign the Region 5 Equipment Use form and submit to regional staff prior to equipment pick-up. Submission may be electronic or hard copy.
